What if I don’t have insurance/my insurance plan is out-of-network?If you do not have in-network health insurance, or are uninsured, you will select “Out-of-Pocket” during registration. You will be asked to pay $55 per flu vaccine and $200 per COVID vaccine up front, unless your school or employer has agreed to cover the cost of your immunization(s). To avoid any fees, out-of-network patients are encouraged to receive their vaccinations from an in-network provider if school or employer sponsorship is unavailable at the SVNA clinic.

Which health insurance plans do you accept?SVNA is in-network with most major health insurance plans except Cigna, TriCare, and certain out-of-state Blue Cross Blue Shield plans. If you have in-network health insurance, you must provide that information during registration. We will then bill your insurance plan for your immunization(s) following the clinic and you will not incur any out-of-pocket fees. If you have questions about insurance, we encourage you to email us at info@seattlevna.com prior to registering for your appointment.

Are there any age requirements?SVNA is licensed to provide flu vaccines to all individuals ages 4 years and older and COVID vaccines to all individuals ages 5 years and older. Age 5-11 COVID vaccines are available at school clinics only, unless agreed upon with the SVNA team.

How do I make an appointment?You will be sent a unique registration link from your school or employer where you may schedule an appointment at your specific clinic. Appointments are available for scheduling in 15-minute increments during clinic hours. All patients will receive a confirmation email and a text message reminder for their appointment.

What happens if I miss my appointment? Can I cancel or reschedule?If you cannot make your appointment, or miss your appointment, please cancel or reschedule via the confirmation email sent at the time of registration.

Can I bring family members or other people with me for my appointment?Family members and friends are welcome to accompany you to your appointment, but they will not receive an immunization unless they are registered themselves. All patients receiving an immunization must meet eligibility requirements and have their own separate registration in their name, with their information. Please inquire with your school or employer if family and friends are permitted to receive vaccinations at your designated on-site clinic.

What do I need to have with me for my appointment?For expedited service, please bring your unique confirmation code (included in your confirmation email), insurance card, and ID to your appointment. Please wear short-sleeved clothing as privacy screens are not included.

Which vaccines do you provide?At our 2025 clinics, we will provide the seasonal Flucelvax Trivalent Preservative-Free flu vaccine and the 2025-2026 Moderna COVID vaccine (pending FDA and CDC approval). We do not offer the Flumist Nasal Spray flu vaccine or the Fluzone High-Dose flu vaccine, nor do we offer Pfizer or Novavax COVID vaccine products.

Can you provide verification of the vaccination given?For official proof of your flu vaccination, please email us at info@seattlevna.com and we will send you a Certificate of Immunization. For official proof of your COVID vaccine, please visit waverify.doh.wa.gov.

What is SVNA’s method of payment for onsite vaccine clinics?SVNA typically generates 95% of revenue through direct insurance reimbursement and 5% of revenue through client/patient pay at workplace events. Our comprehensive network of insurance contracts is designed to make our services as accessible and cost-effective as possible for our clients – we are in-network with most major insurance plans except Cigna, TriCare, and certain out-of-state BlueCross BlueShield plans. Onsite vaccine clinics are offered at no cost to our clients unless the client elects to pay for uninsured and out-of-network patient vaccinations.

My organization’s insurance plan is out-of-network. How do we proceed and what are the costs?Your organization may elect to be direct-billed by SVNA for the cost of uninsured or out-of-network patient vaccinations at a rate of $55 per flu vaccine and $200 per COVID vaccine, if applicable. Alternatively, uninsured or out-of-network patients can pay for their vaccines individually. SVNA is committed to working with your organization to maximize accessibility for all participants.

How do I schedule an onsite vaccine clinic at my workplace?To schedule an onsite vaccine clinic at your workplace, please fill out our short questionnaire via JotForm so we can get an idea of your organization’s needs. Once received, our scheduling team will reach out to your designated contact person with a clinic proposal. We will release this season’s JotForm starting April 1st, 2025, on the SVNA homepage.

My onsite vaccine clinic is confirmed. How will participants sign up?Your organization will receive an online registration link no later than 3 weeks prior to your clinic. Participants will use the registration link to consent to our services, fill in demographic and insurance information, and select a 15-minute appointment window during clinic hours. All patients MUST be registered for the clinic in advance to receive their vaccination(s).

Can family members and dependents attend our onsite vaccine clinic? Are there age requirements?Yes – we are happy to provide vaccinations to your staff’s family members and dependents if your organization permits them to attend. All patients receiving a vaccination must meet eligibility requirements and have their own separate registration, in their name, with their information. Family members and dependents must have in-network insurance or agree to pay out-of-pocket for their vaccinations, unless your organization has chosen to sponsor uninsured/out-of-network participants. SVNA offers flu vaccines to patients ages 4 and older and COVID vaccines to ages 12 and older at our corporate events. Age 5-11 COVID vaccines are available at school clinics only, unless agreed upon with the SVNA team.

What supplies/materials are needed to host an onsite vaccine clinic?We ask our clients to reserve a large space within their workplace for the day of the clinic, as well as provide adequate tables and chairs for our staff and clinic participants. SVNA will coordinate with your designated contact regarding spatial and table/chair requirements as part of the scheduling process, commensurate with expected volume. Our nurses will provide all medical supplies needed for vaccine administration, including sharps containers for pharmaceutical waste which our staff will dispose of after the clinic concludes.

How many SVNA staff members will attend my clinic? What are their credentials?The number of nurses who attend your clinic will depend on the expected volume. SVNA will staff your clinic on a fluid basis as we monitor the number of registrations that come in during the weeks prior to your scheduled event. Our nurses are all W2 employees, licensed to practice nursing in the State of Washington and have years of immunization experience. For larger volume clinics, SVNA will also send a Site Manager to direct foot traffic, assist with check-in, and help answer patient questions.

Will SVNA provide marketing materials for promotion of my clinic?Yes - in addition to a registration hyperlink, our scheduling team will provide your designated contact with a helpful FAQ document and a registration QR code flyer that can be easily shared electronically and/or physically with your staff. We encourage our clients to promote their clinic with internal newsletters, emails, and memos in conjunction with the marketing materials we provide.

How will participants know if their health insurance plan is in-network with SVNA?Our school-based vaccine clinics are typically large-scale events where we see patients with a wide range of health insurance plans. During registration, patients will select their insurance plan from a drop-down menu and will be prompted to enter their Member ID number and Group Number. Our software system will then verify whether the patient’s plan is in-network. Patients whose insurance plan cannot be verified, or who don’t see their insurance plan listed in our drop-down menu, are encouraged to email us at info@seattlevna.com prior to continuing with registration.

What are the costs for uninsured/out-of-network patients at school-based vaccine clinics?Uninsured and out-of-network patients who still wish to participate will select “Out-of-Pocket” during registration. They will be asked to pay $55 per flu vaccine and $200 per COVID vaccine up-front to receive their vaccination(s). That said, we highly encourage out-of-network patients to seek out an in-network provider to receive their vaccinations at no cost (e.g. local pharmacies or their primary care provider).

How do I schedule an onsite vaccine clinic for my school and/or wider district?To begin the scheduling process, please fill out our short questionnaire via JotForm (available starting April 1st, 2025, on the SVNA homepage). Once received, our scheduling team will reach out to your designated contact person with a clinic proposal. For large school districts, we will typically coordinate a series of after-school and/or weekend clinic events at a variety of different school locations. This structure is designed to capture as many participants as possible and make the clinics more convenient for families and working parents.

Advance registration is required, and all patients must have their own separate registration, in their name, with their information (e.g. a family of four will require four separate registrations).

Parents and guardians with minor children will sign a consent form on their child’s behalf during registration and select the vaccine(s) for each patient based on age-group.

What logistical considerations are required to host a school-based vaccine clinic?We typically have the most success with large-volume school clinics when hosted in a cafeteria or gym where there is ample space to ensure smooth operations. We will ask the host school to provide adequate tables and chairs for our staff and clinic participants, as well as trash cans for non-hazardous waste. SVNA will coordinate with your designated contact regarding spatial and table/chair requirements as part of the scheduling process, commensurate with expected volume at each clinic. Our nurses will provide all medical supplies needed for vaccine administration, including sharps containers for pharmaceutical waste which our staff will dispose of after the clinic concludes. SVNA requires bathrooms and accessible parking to be available for the duration of the event.

SVNA will also send one or two Site Managers to all school-based clinics to direct foot traffic, assist with check-in, and help answer patient questions. A school staff member will need to be available to unlock the building and direct our team to the clinic space for set-up, and to lock and secure the building when the clinic concludes.
